When we agree upon an initial appointment time I will email you links to these forms or you can access them from this website. The Disclosure form is easiest to complete when using a touch screen device like your cell phone or tablet. Simply sign with your finger. The video conferencing platform we will be using is HIPAA compliant and is very user friendly. This platform does allow for sessions to be transmitted from multiple sources, meaning you and your partner can be in separate locations, if needed. You both will receive the link the morning of your appointment by email or text.
